Thank you for going ‘that extra mile’ to help a child or young person; your time is precious, the following is a guide to the straightforward process of achieving financial help for them. The person completing the referral form needs to hold one of the following positions:

MEDICAL • Consultant Paediatrician • General Practitioner • Health Visitor or C.S.N • Paediatric or specialist Physiotherapist • Occupational Therapist • Senior Portage Home Visitor • School Nurse • Medical Social Worker • Speech & Language Therapist

EDUCATIONAL Nursery/Pre-school – Leader/Manager (suitably qualified), SENCO; Primary or Secondary – Head teacher, Deputy Head teacher or SENCO; Special Schools – Head teacher, Deputy Head teacher, class teacher or Therapy Manager; Educational Psychologist

SOCIAL SERVICES • Paediatric Social worker • Family Link Co-ordinator • Children’s Team Manager • Disabled children’s Team manager

How do we proceed?

Speak to the family to ensure they are happy for you to make a referral. The referral form can be completed on line or printed off and completed by hand. The form needs to be signed and posted to the referrals secretary at the address on the bottom of the form. What happens next?

The referrals secretary will allocate the child a reference number, carefully summarize the application details and circulate this data to the committee and trustees by email. All details will remain absolutely confidential. If anyone has any questions, thinks more information is needed or suggests other statutory sources of funding, then the referrals secretary will make contact with the person completing the form. Once SKF has enough information to make a decision a vote is conducted and the referrals secretary will inform the referrer of that decision. The equipment or lessons are then booked, by referrer or parent. It is very important that everyone understands that payment is only made to the provider – we do not refund or pay individuals and we do not make retrospective payments. For a very straightforward application a decision will normally be made within 2 weeks; more complex applications may take longer.

Can we apply more than once?

Yes. Many of the children we have helped over the years with developmental issues, chronic medical or educational problems will have different needs and requirements as they grow up.

What sort of things can SKF help fund?

It is difficult to provide a complete list as needs and equipment change quickly and children are all different! So, what ever it is just ask; you can email your enquiry to our referrals secretary who will be able to tell you if it is a request we would be able to consider.
